How it works
Four steps to seek reimbursement
Many PPO plans include out-of-network benefits. Whether your plan reimburses a visit, and how much, depends on your policy, your deductible and the service.
- Check your benefitsCall the number on your insurance card and ask about out-of-network coverage for the service you plan to book. Our list of questions below helps.
- Pay at your visitPay for your appointment as usual. HSA, FSA, HRA and LSA cards are welcome. Your plan decides which services are eligible.
- Request your superbillAsk our team for a superbill for your medical visits. It lists the details your insurer needs to process a claim.
- Submit your claimSend the superbill to your insurer through their member portal, app or claim form, then follow up on the status of your claim.
What a superbill is
An itemized receipt your insurer can read
A superbill is a detailed statement for medical services you have already paid for. Insurers use it to decide whether to reimburse part of your cost under your out-of-network benefits.
Superbills are available for eligible medical services provided by our licensed medical team through Kasawa Medical APC, dba Saffron & Sage MD. Non-medical wellness services are generally not billed to insurance. Ask us which of your visits qualify for a superbill.
A superbill typically includes
- Your name and date of birth, and the date of each visit
- The treating provider's name, credentials and NPI number
- The practice name, address and tax ID
- Diagnosis codes (ICD-10) that describe the reason for your visit
- Procedure codes (CPT) that describe the service you received
- The amount charged and the amount you paid
Letter of Medical Necessity
When a letter can help
Some insurers, and many HSA and FSA plans, ask for a Letter of Medical Necessity (LMN) before they will cover certain services or products. An LMN is written by your provider and explains why a treatment is recommended for a diagnosed condition.
When it is appropriate for your care, our medical team can provide one. Not every service qualifies, and your plan administrator makes the final decision.

- Is there a limit on the number of covered visits per year?
- Do I need a referral, pre-authorization or a Letter of Medical Necessity?
- How do I submit a superbill, and how long do I have to file a claim?

Do you accept insurance?
No. Saffron & Sage is a cash practice and does not bill insurance companies directly. We provide superbills for eligible medical services so you can submit them yourself for possible out-of-network reimbursement.
Will my insurance reimburse me?
It depends on your plan. Many PPO plans offer out-of-network benefits, while most HMO plans do not. Coverage varies by service, deductible and policy, so we recommend calling your insurer before your visit. We cannot guarantee reimbursement.
